Establishes the Help Me Grow program as a comprehensive access, information, referral, and follow-up system for pregnant and parenting families with children from birth to age 8.

Commissioner of education shall provide funding and work cooperatively with commissioners of human services and health to implement the program through interagency agreements.

Program shall facilitate collaboration across child health, early learning and education, and family supports by providing health care provider outreach, identifying detection tools for developmental and behavioral risks, and linking families to community-based services.

Requires development of a centralized access point for parents and professionals and a resource directory including medical care providers, child care programs, mental health services, developmental disability programs, and family support services.

Departments of Education, Health, and Human Services must annually review program outcomes, alignment with early childhood goals, and impacts on young children, and collect data on service gaps and barriers.
