Amends Minnesota Statutes 2016, section 62A.30 to require health insurance coverage of digital breast tomosynthesis as part of preventive mammogram screening for enrollees at risk for breast cancer.

Defines "digital breast tomosynthesis" as a radiologic procedure producing cross-sectional digital three-dimensional images of the breast.

Establishes four criteria for "at risk for breast cancer": family history with one or more first or second degree relatives with breast cancer, positive BRCA1 or BRCA2 mutations, heterogeneously or extremely dense breasts per American College of Radiology standards, or previous breast cancer diagnosis.

Excludes coverage provided through public health care programs under chapters 256B or 256L from these requirements.

Effective January 1, 2019 for health plans issued, sold, or renewed on or after that date.
