Requires the notification center to create and maintain a database of contact information for all operators in Minnesota responsible for underground facilities.

Database must include operator name, telephone number, mailing address, and email address (if available) for emergency response contacts available 24 hours per day, seven days per week.

Contact information must be annually updated to ensure current accuracy.

Database information must be made available to excavators upon request.

Amends Minnesota Statutes 2016, section 216D.03 by adding a new subdivision.
