Amends Minnesota Statutes section 144.586 to add care coordination requirements for hospital discharges of children with high-cost medical or chronic conditions requiring post-hospital continuing aftercare.

Requires hospitals to notify the patient's primary care provider and health carrier or managed care organization of discharge date and provide a copy of the discharge plan and aftercare needs before discharge.

Mandates that appropriate arrangements for home health care or post-hospital extended care services be made and initial services indicated on the discharge plan be scheduled prior to discharge.

Requires hospitals to connect patients eligible for care coordination services through a health plan or certified medical home with the appropriate care coordinator before discharge.
