Dental plan organizations must disclose the fee schedule by procedure code for each procedure to be performed so providers can determine expected reimbursement.

Contracted providers receiving dental plan fee schedules are prohibited from releasing the plan's aggregate maximum allowable fee table in any manner that would violate state or federal antitrust laws.

Health plan companies must provide providers with a complete contract copy before signing, including all attachments, operating manuals, coding guidelines, and treatment parameters.
