Preserves specific age-based exemptions for pertussis (age 7+), poliomyelitis (age 18+), measles/rubella/mumps (under 15 months), and haemophilus influenzae type b (age 5+).

Updates school and child care facility reporting requirements to track exemptions granted under medical or religious grounds only, removing references to philosophical exemptions.
