Amends Minnesota Statutes section 151.214 to require pharmacists to provide patients with co-payment amounts, the pharmacy's usual and customary price, and the amount the pharmacy will be paid for each prescription covered by employer-sponsored or health plans.

Requires health plans and pharmacy benefit managers to provide pharmacies with necessary data during initial claim adjudication to comply with disclosure requirements.

Prohibits contractual agreements between health plans, pharmacy benefit managers, and pharmacies from preventing pharmacies from disclosing required pricing information to patients.

Prohibits health plans and pharmacy benefit managers from contractually preventing pharmacists from informing patients when their health plan co-payment for a drug exceeds the out-of-pocket cost at the pharmacy's usual and customary price.
