Health carriers must notify affected providers and enrollees at least 60 days before changing a provider's status from in-network to out-of-network, using U.S. mail or electronic mail.

Health carriers must notify only enrollees who have used that provider within the previous 12 months from the date notification is required.

Health carriers must update their websites at least once per month with provider network changes after the required notification has been provided.

Health carriers must reprocess claims as in-network claims if services were provided after a network status change took effect but before the change was posted online, unless the carrier notified the enrollee before service was provided or can verify the website displayed correct status at the time of service.

Limitations in Minnesota Statutes section 62Q.56, subdivision 2a, apply to payments required under this section.
