Expands the definition of "external fixed costs" to include rate add-ons for border city facilities under section 256R.481.

Allows nonprofit nursing facilities in Breckenridge or Moorhead (existing prior to January 1, 2015) to apply annually for a rate add-on to their external fixed costs payment rate.

Sets the rate add-on amount equal to the difference between the median total payment rate for case mix classification PA1 in adjacent out-of-state cities and the facility's own PA1 rate.

Requires facilities to submit applications within 60 calendar days of the effective date, with commissioner authority to waive deadlines under extraordinary circumstances.

Repeals the previous facility-specific exemption for the Breckenridge nursing facility in section 256R.53, subdivision 2, effective December 31, 2020, with the new add-on mechanism available beginning January 1, 2021.
