Requires health plan companies offering coverage in southeastern Minnesota to establish a shared savings incentive program from January 1, 2020, through January 1, 2022, that rewards enrollees for using lower-cost providers for comparable health care services.

Health plans must provide enrollees at least 50 percent of the savings when they receive covered services from providers charging less than the plan's average allowed amount, with no payment required if savings are $25 or less.

Establishes a shared savings incentive account for each enrollee where incentive payments are deposited and can be withdrawn to pay cost-sharing or premiums, and creates a state web-based tool to compare provider charges by procedure code.

Allows enrollees to use out-of-network providers at lower costs with deductible and out-of-pocket maximum credit applied as if services were in-network, and requires health care providers to post notifications about price transparency and comparison options.

Commissioner must receive annual reports by March 1, 2021 and 2022 on incentive payments, service categories used, savings achieved, and enrollee participation, with aggregate reports submitted to legislature by April 15.
