Mandates comprehensive evaluations of the health care home model at three years and five years after implementation.

Requires reports to identify state health care program enrollees with complex or chronic conditions by income, disability, race, ethnicity, and language.

Includes analysis of health care home provider distribution, care quality, preventive care measures, payment arrangements, implementation costs, health disparities impact, and estimated savings across fee-for-service, managed care, and county-based purchasing sectors.
