Requires municipalities, businesses, nonprofit organizations, and schools that charge activity fees for youth athletics to obtain a medical eligibility form signed by a health care provider for each participating youth athlete.

The eligibility form must document that the youth athlete has been screened for conditions associated with sudden cardiac arrest and assessed with a double-cuff blood pressure measurement device.

Maintains existing concussion protocol requirements including coach and official training on concussion recognition and management, with refresher training required at least once every three calendar years.

Requires coaches and officials to remove youth athletes from activity if they exhibit signs, symptoms, or behaviors consistent with a concussion, with return to play only after medical evaluation and written clearance.

Provides liability protection for municipalities, businesses, nonprofit organizations, and their officers, employees, and volunteers from additional liability or legal action related to these requirements.
