Requires patients to provide evidence of receiving payments from or participating in one of the specified programs to qualify for the reduced $50 fee.

Defines veterans disability payments to include VA dependency and indemnity compensation.

Establishes that SSDI recipients transitioning to retirement benefits by Social Security Administration are still considered SSDI recipients for fee purposes.

Maintains annual fee payment structure due on the anniversary date of patient enrollment, with fees deposited to the state government special revenue fund.
