Amends Minnesota Statutes section 256B.49, subdivision 14, regarding assessment and reassessment requirements for home and community-based service waivers for persons with disabilities.

Allows current service providers to submit written reports with care recommendations prepared by direct service employees familiar with the person, with submission required at least 60 days before the end of the current service agreement, and requires certified assessors to consider these reports.

Requires only face-to-face assessments conducted according to specified sections that determine hospital or nursing facility level of care to be accepted for initial and ongoing access to waiver services payment.

Permits recipients found eligible for home and community-based services before age 65 to remain eligible after age 65 if they continue to meet all other eligibility factors.

Prohibits termination of home and community-based services waivers when recipients are placed in hospitals, nursing facilities, or treatment programs for at least 30 days if there is reason to believe they can eventually return to the community; requires immediate reinstatement of previously eligible services upon discharge.
