Commissioner must provide medical assistance coverage of health home services for eligible individuals with chronic conditions who select a designated provider, with implementation complying with federal requirements under United States Code, title 42, section 1396w-4.

Establishes behavioral health home services for individuals eligible for medical assistance who meet the definition of mental illness or emotional disturbance, focusing on both behavioral and physical health.

Adds medical recuperative health home services for individuals with chronic conditions who lack permanent housing at the time of emergency department or hospital discharge, effective upon federal approval.

Allows discharge from behavioral health home services only if the provider cannot locate or engage the individual for more than three months, or if the individual refuses to participate, with required face-to-face meeting before discharge.

Requires commissioners to establish designated providers as health homes, apply for federal grants under section 1396w-4(c)(3), and collect data on health home performance against process, outcome, and quality standards.
