Adds a new religious belief exemption to Minnesota's immunization requirements, allowing parents or guardians to submit a signed statement asserting a "genuine and sincerely held religious belief" to exempt their child from required immunizations for school and child care facility enrollment.

Requires schools and child care facilities to forward religious exemption statements to the Minnesota Commissioner of Health, similar to existing requirements for conscientiously held belief exemptions.

Mandates that written immunization information provided by the Department of Health, Department of Education, Department of Human Services, immunization providers, and schools or child care facilities must describe all available exemptions in font size equal to and on the same page as immunization requirements.

Requires immunization providers to give written information before administering vaccines, including a list of required and recommended immunizations, description of exemptions, and federal vaccine information sheets detailing possible adverse reactions.

Updates school and child care facility reporting requirements to include counts of persons receiving exemptions under the medical, conscientious belief, and new religious belief exemption categories.
