Modifies the Department of Human Services incentive-based grants program for customized living service providers to expand eligibility beyond elderly waiver providers to include brain injury and community access for disability inclusion waiver providers.

Establishes preference criteria for grant awards: providers serving at least 75 percent elderly waiver participants OR at least 25 percent brain injury or community access for disability inclusion waiver participants.

Allocates $500,000 in fiscal year 2020 and $500,000 in fiscal year 2021 for the incentive-based grants program targeting improved service efficiency, quality, innovation, and organizational capacity.

Directs the revisor of statutes to codify the modified grant program as Minnesota Statutes, section 256.479, effective July 1, 2020.
