Prohibits employers from disciplining, discharging, or refusing to hire applicants who refuse immunization if the employee or applicant provides a physician-signed statement that the immunization is medically contraindicated or that adequate immunity exists, or provides a statement citing conscientiously held beliefs.

Applies to employers in Minnesota with one or more employees, including state and local government entities, unpaid board members, and volunteers age 18 or older.

Allows employers to establish reasonable policies regarding the timing, form, and process for submitting medical or religious exemption statements from employees or applicants.

Establishes civil action as the sole remedy for violations, with damages limited to lost wages and benefits resulting from the violation.

Requires courts to award court costs and reasonable attorney fees to the prevailing party, whether plaintiff or defendant.
