Requires health insurance coverage of digital breast tomosynthesis (3D breast imaging) for enrollees at risk for breast cancer as a preventive service.

Defines "at risk for breast cancer" to include individuals with family history of breast cancer, BRCA1/BRCA2 mutations, heterogeneously or extremely dense breasts, or previous breast cancer diagnosis.

Exempts public health care programs under chapters 256B and 256L from this coverage requirement.

Allows insurers to cover digital breast tomosynthesis for enrollees not meeting the "at risk" criteria and does not limit coverage under policies in effect before January 1, 2020.

Takes effect January 1, 2020, and applies to all health plans issued, sold, or renewed on or after that date.
