HF 4520 Summary

• Expands long-term care options counseling services by establishing new "long-term care options counseling" distinct from in-person "long-term care consultation services," with counseling provided through Senior LinkAge Line and Disability Linkage Line partnerships.

• Requires hospitals to refer at-risk individuals to Senior LinkAge Line for long-term care options counseling prior to discharge, and mandates prospective residents of housing with services establishments receive and verify options counseling before signing leases or contracts.

• Establishes four exemptions from options counseling requirements: lease-only arrangements in subsidized housing, prior long-term care consultation assessments, hospice service eligibility, and recent financial planning with long-term care plan creation.

• Modifies long-term care consultation team requirements, including certified assessor qualifications, training and certification processes through MnCHOICES curriculum, and establishes eligibility criteria for consultation services access.

• Repeals former subdivisions 3c, 3d, and 3e of section 256B.0911 and updates payment methodologies, allowing direct commissioner reimbursement to counties and tribal nations for long-term care consultation services effective July 1, 2020.
