Modifies Minnesota Statutes section 62J.701 to apply health plan requirements to governmental programs (medical assistance, MinnesotaCare, state employee group insurance, public employees insurance, and political subdivision coverage) effective January 1, 2021.

Requires health care providers to include on patient bills the dollar amount the provider will accept as full payment, the Medicare-allowable fee-for-service payment rate, and the provider's "Medicare percent" (percentage of Medicare allowable rate the provider accepts as full payment).

Establishes new section 62J.84 requiring health care providers to establish and publicly disclose a "Medicare percent" through posted notices at reception/check-in areas, on their website, and on payment documents patients sign.

Requires providers to obtain signed Medicare percent disclosure forms from patients before providing services, with $1,000 fines for non-compliance; providers failing to obtain signatures face reimbursement limited to Medicare-allowable rates or their disclosed Medicare percent.

Applies the hospital or clinic's Medicare percent to all individual providers employed by, affiliated with, or contracted with that facility, preventing higher reimbursements than the facility's established rate.
