Requires health care providers to determine whether proposed services are covered by a consumer's health plan before providing services and notify the consumer in writing if any services are not covered.

Prohibits providers from billing consumers for noncovered services unless the provider obtains the consumer's signed acknowledgment that the services are not covered.

Requires providers to disclose whether they participate in the consumer's health plan provider network and obtain signed acknowledgment from consumers if the provider is out-of-network.

Limits out-of-network provider billing to amounts authorized in the consumer's out-of-network fee schedule if the provider fails to obtain required signed acknowledgment.

Allows consumers to receive noncovered services from a provider only after the provider has complied with disclosure and signature requirements and obtained consumer acknowledgment.
