• Commissioner must provide medical assistance coverage of health home services for eligible individuals with chronic conditions and establish health homes for populations with serious mental illness.

• Individuals are eligible for behavioral health home services if they are eligible for medical assistance and have a condition meeting the definition of mental illness or emotional disturbance under Minnesota law.

• Individuals are eligible for medical recuperative health home services if they have a chronic condition as defined in federal law and lack permanent residence at the time of discharge from an emergency department or hospital.

• Behavioral health home services providers must offer a face-to-face meeting with the individual and their identified supports before discharge, and can only discharge individuals after three months of inability to locate or engage them, or if the individual refuses to participate.

• Commissioner must establish designated providers as health homes, apply for federal grants under the Affordable Care Act, and collect data to monitor compliance with process, outcome, and quality standards.
