Modifies incentive-based grant program for customized living service providers under brain injury, community access for disability inclusion, and elderly waivers, expanding eligibility beyond the original fiscal years 2020 and 2021 limitation.

Until June 30, 2021, grants prioritize providers serving at least 75 percent elderly waiver participants; effective July 1, 2021, providers must serve at least 75 waiver participants with 75 percent of their clients being waiver participants.

For brain injury or community access for disability inclusion providers, the required 75 waiver participants must reside at multiple locations with at least six or more residents per location.

Appropriates $500,000 in fiscal year 2020 and $500,000 in fiscal year 2021 for the incentive-based grant program.

Codifies the grant program as Minnesota Statutes, section 256.479, with all sections effective July 1, 2020.
