Adds a new religious belief exemption to Minnesota's immunization requirements, allowing parents or guardians to submit a notarized statement that a child has not been immunized due to a "genuine and sincerely held religious belief" to exempt children from school and child care facility immunization requirements.

Requires written information about immunization exemptions to be provided by the Department of Health, Department of Education, Department of Human Services, immunization providers, and schools or child care facilities in font size equal to immunization requirements and on the same page.

Mandates immunization providers give written information before vaccinating, including a list of required immunizations, description of exemptions, additional recommended immunizations, and federal vaccine information sheets listing possible adverse reactions.

Directs the health commissioner to continue educational campaigns on vaccine safety, encourage providers to distribute vaccine information statements at multiple visits, and encourage use of thimerosal-free vaccines when available.

Modifies school and child care facility reporting requirements to include counts of persons receiving exemptions under the new religious belief exemption category alongside existing medical and conscientious belief exemptions.
