Modifies service plan review requirements for waiver providers to include annual meetings discussing technology use, residential transition options, and day service employment transitions, with summaries documented in coordinated service and support plan addendums.

Expands long-term care consultation services to include information about independent living, self-directed services and supports, and eligibility determination for additional state plan services including personal care assistance, consumer support grants, and semi-independent living services upon statewide lead agency implementation.

Requires certified assessors and case managers to conduct person-centered planning ensuring individuals are fully informed of all available employment, living arrangement, and self-directed service options before needs assessment and plan development.

Directs the Commissioner of Human Services to update the MnCHOICES assessment tool by January 1, 2021, and implement procedures for person-centered planning, informed decision-making, inter-assessor reliability training, and continuous process improvement using Lean Six Sigma methodology.

Modifies case management activities to explicitly include assisting individuals in identifying providers of nondisability-specific services, employment service providers, and financial management service providers.
