Establishes $20,305,000 in coronavirus relief fund appropriation for COVID-19-related retention grants to eligible home and community-based service providers to cover business interruptions and maintain service access, with grants equal to 66 percent of each provider's January 2020 medical assistance revenue.

Creates $10,125,000 in coronavirus relief fund appropriation for disability services providers to implement COVID-19 public health measures that reduce congregate settings and expand individualized day and employment services, with grants up to $200,000 per provider.

Modifies customized living quality improvement grant eligibility requirements effective July 1, 2021, requiring providers to serve at least 75 waiver participants with 75 percent of clients being waiver participants, with additional location requirements for brain injury and community access providers.

Extends COVID-19 emergency waivers and modifications through June 30, 2021, including telemedicine services, remote case management, flexible licensing for substance use disorder treatment, and economic assistance program provisions, except verbal signatures are prohibited for enrollment verification.

Appropriates $500,000 each for fiscal years 2020 and 2021 for incentive-based grants to elderly waiver, brain injury, and community access for disability inclusion customized living service providers.
