Modifies customized living quality improvement grants for elderly waiver and disability waiver service providers, requiring providers to serve at least 75 waiver participants with 75 percent of clients being waiver participants effective July 1, 2021, with $500,000 appropriated for fiscal years 2020 and 2021.

Extends COVID-19 peacetime emergency waivers and modifications to human services programs through June 30, 2021, including telemedicine services, remote service delivery, and flexibility in licensing requirements for substance use disorder treatment and mental health centers.

Appropriates $20,305,000 from the coronavirus relief fund for COVID-19-related retention grants to home and community-based service providers covering fixed costs from March 1 through December 30, 2020, with grants equal to 66 percent of providers' January 2020 medical assistance revenue.

Appropriates $10,125,000 from the coronavirus relief fund for COVID-19-related public health grants to disability services providers implementing social distancing measures, with maximum grants of $200,000 per provider and priority given to providers demonstrating greatest need for individualized day or employment services.

Requires commissioner reporting on public health grant utilization by March 1, 2021, with legislative report due August 1, 2021, detailing total funds allocated, uses, outcomes, and people impacted.
