Establishes a one-time insulin registration fee of $3,000,000 total assessed to manufacturers based on their percentage of insulin units reported in calendar year 2019, with individual fees due by March 1, 2021.

Creates an insulin assistance account in the special revenue fund to receive registration fees and collect penalties, with funds appropriated to reimburse manufacturers for insulin dispensed under the insulin safety net program.

Modifies the Alec Smith Insulin Affordability Act by allowing manufacturers to request reimbursement from the commissioner of administration for insulin payments made to pharmacies and for insulin provided through the continuing safety net program.

Requires manufacturers and wholesalers to report all insulin sales and distributions to practitioners and pharmacies by September 1, 2020, with non-compliance penalties up to $10,000 per day deposited in the insulin assistance account.

Transfers $3,000,000 from the health care access fund to the insulin assistance fund in fiscal year 2021, with any surplus at the end of fiscal year 2021 refunded proportionally to manufacturers.
