Modifies customized living quality improvement grants for elderly waiver and brain injury/community access for disability inclusion providers, requiring 75 waiver participants and 75 percent waiver participation eligibility starting July 1, 2021.

Extends 15 COVID-19 emergency waivers and modifications through June 30, 2021, including telehealth services, remote case management, flexible housing licensing, and remote adult day services.

Appropriates $20,305,000 from coronavirus relief fund for COVID-19 retention grants to home and community-based service providers for fixed costs incurred March 1 through December 30, 2020, calculated at 66 percent of January 2020 medical assistance revenue.

Appropriates $10,125,000 from coronavirus relief fund for disability services provider public health grants to reduce COVID-19 exposure risks, with maximum grants of $200,000 per provider for social distancing measures and individualized services.

Authorizes commissioner to request federal waivers and appendix K amendments to provide emergency funding to home and community-based providers affected by the COVID-19 public health emergency.
