Establishes Primary Care Case Management (PCCM) program to improve health outcomes and reduce health care costs by enabling direct state payments to health care providers for medical assistance and MinnesotaCare enrollees.

Authorizes the commissioner to stop renewing managed care plan contracts under Minnesota Statutes sections 256B.69 and 256L.12 for medical assistance and MinnesotaCare services.

Permits enrolled patients to select primary care providers (physicians, nurses, specialists, or certified case management professionals) to coordinate care for those with specific or complex medical conditions.

Requires the commissioner to set case management fees based on care coordination complexity and socioeconomic factors, and to provide 24-hour nurse consultation helplines, provider locator services, and preventive care outreach to enrollees.

Direct payments to providers become effective when current managed care plan contracts expire; the bill takes effect the day following final enactment.
