Appropriates funds in fiscal year 2021 from the general fund to the Commissioner of Human Services for grants to eligible providers of home and community-based services to address COVID-19 business interruptions, increased costs, and ensure service continuity.

Defines eligible providers as enrolled providers offering residential services through home and community-based waiver programs or services through intermediate care facilities for persons with developmental disabilities, based on 2019 medical assistance revenue.

Grants providers 2.5 percent of their total medical assistance revenue from calendar year 2019 for eligible services, with funds usable only for unreimbursed costs or losses incurred from March 1, 2020, through December 30, 2020.

Requires eligible providers to attest to intent to continue providing services, document unreimbursed COVID-19 related costs, and agree to cooperate with the Commissioner and maintain supporting documentation.

Authorizes the Commissioner to audit providers up to six years after grant award and recoup any amounts used for unauthorized purposes; all provisions expire December 31, 2021, except recoupment authority.
