Requires the commissioner to develop and implement a uniform outcome measurement and reporting system for state-funded homelessness programs, including emergency services grants, long-term homeless supportive services, and Homeless Youth Act grants.

Defines "exit" as not receiving shelter or supportive services for 90 consecutive calendar days, with the exit date being the last date of service.

Mandates the commissioner report biannually (by December 31 of even-numbered years) to legislative committees on program outcomes including total clients served, program costs, cost per client, changes in homelessness rates, housing opportunities, employment outcomes, and emergency service usage.

Requires grant recipients to submit data to the commissioner by September 1 of even-numbered years in a format prescribed by the commissioner, with technical assistance provided beginning July 1, 2022.

Makes all collected data publicly available on the department's website with analysis in written and graphic formats; programs existing as of May 1, 2022 become ineligible for additional appropriations after June 30, 2023 unless reporting requirements are satisfied.
