Defines advance care planning services as discussions between a health professional and patient about the patient's healthcare wishes, including future healthcare decisions, health care directives, and completion of standard health care directive forms

Specifies that qualified health professionals include physicians, physician assistants, nurse practitioners, clinical nurse specialists, or other medical professionals working under direct physician supervision

Mandates that medical assistance (Minnesota's Medicaid program) covers advance care planning services effective January 1, 2023

Applies the same definitions used for health plans to medical assistance coverage requirements
