Expands conscientious belief exemptions from immunization requirements to allow parents, guardians, or emancipated persons to submit notarized statements citing conscientiously held beliefs for children in schools and child care facilities, and for postsecondary students.

Adds language clarifying that exemptions apply to immunizations "otherwise required for enrollment" in schools, child care facilities, and postsecondary institutions, not just statutorily prescribed immunizations.

Requires institutions to forward notarized conscientious belief exemption statements to the Minnesota Department of Health commissioner for tracking purposes.

Effective immediately upon final enactment and applies to all immunization requirements in effect on or after that date.

Applies to exemptions in Minnesota Statutes sections 121A.15 (schools and child care) and 135A.14 (postsecondary institutions).
