Creates "Choose Life" special license plates available for passenger automobiles, noncommercial one-ton pickup trucks, motorcycles, and recreational vehicles, with plate design including the "Choose Life" kids logo and a parent-child silhouette option by January 1, 2024.

Requires applicants to pay special plate fees, registration tax, and contribute $25 upon initial application plus a minimum of $25 annually to the choose life account.

Establishes the choose life account in the special revenue fund with contributions appropriated to the commissioner of public safety, with administration costs deducted and remaining funds distributed to Choose Life Minnesota, Inc.

Directs Choose Life Minnesota, Inc. to distribute funds by county to nongovernmental nonprofit agencies assisting pregnant women making adoption plans, excluding agencies involved in abortion activities or charging for services.

Restricts fund use to material needs (clothing, housing, medical care, food, utilities, transportation) and post-delivery support for 60 days, with Choose Life Minnesota, Inc. limited to 15 percent of funds for administration and promotion.

Effective January 1, 2023, for special plates issued on or after that date.
