Establishes a competitive grant program administered by the Commissioner of Human Services for nonprofits serving homeless youth aged 18-24 who are living with family friends, relatives, or neighbors to avoid homelessness.

Requires the commissioner to contract with a technical assistance provider located in Hennepin County to develop grant criteria, select grantees, provide technical assistance, and conduct annual evaluations of program efficacy and cost-effectiveness.

Mandates that all grantees partner with at least one youth-host pair, mediate living arrangement agreements, provide monthly stipends to offset costs, and connect participants to community resources.

Requires grants to be awarded to at least five nonprofit applicants beginning no later than December 31, 2022.

Appropriates $8,000,000 from the general fund in fiscal year 2023 for the grant program, available until June 30, 2027.
