Adds "care coordination" and "care evaluation" as new covered home care services performed by licensed professionals employed by home health agencies to coordinate and assess skilled and unskilled home care services.

Limits care coordination services to up to four 15-minute units per episode of care and includes care evaluations within the existing limit of up to nine face-to-face skilled nurse visits per calendar year.

Establishes homemaker service rate methodology beginning January 1, 2023, using wage index calculations under Minnesota Statutes sections 256S.211-215, with wage data updates on November 1, 2024, and every two years thereafter.

Applies homemaker rate methodology to services provided under home and community-based services waivers, alternative care, essential community supports, and elderly waiver programs.

Clarifies that Medicare administrative nursing visits are noncovered services, with the exception of "care evaluation" visits as newly defined in the bill.
