• Long-term care consultation teams must visit persons requesting assessment within 20 calendar days after an assessment is requested or recommended.

• Lead agencies must use certified assessors to conduct assessments, with public health or registered nurses consulted for persons with complex health care needs.

• Assessments must be comprehensive, conversation-based, and person-centered using the MnCHOICES assessment tool, evaluating health, psychological, functional, environmental, and social needs.

• Community support plans and coordinated service plans must be completed within 60 calendar days of the assessment visit and must include assessed needs, service options, health and safety risk identification, and information about self-directed services.

• Remote reassessments by video or telephone may substitute for face-to-face reassessments under specified conditions with agreement from the person and lead agency, though developmental disabilities and elderly waiver programs have different remote reassessment frequency limits.
