Directs commissioner of health to establish eight regional multidisciplinary overdose prevention teams geographically aligned with emergency medical services regions to implement community-specific prevention programs and policies.

Requires commissioner to establish a community-based grant for the Homeless Overdose Prevention Hub to provide emergency and short-term housing subsidies for homeless individuals at risk of overdose, primarily serving urban American Indians in Minneapolis and St. Paul through the Native American Community Clinic.

Establishes a grants and contracts program to strengthen the recovery-friendly workplace initiative, which helps create work environments that prevent drug abuse, provide employer training, and reduce stigma for people in recovery.

Requires evaluation of program components using measures including mortality, morbidity, homelessness, workforce wellness, employee retention, and program reach, with grantees reporting outcomes to the commissioner.

Amends Minnesota Statutes section 145.924 to authorize grants to agencies experienced in syringe services programs to expand access to harm reduction services and improve care preventing HIV/AIDS, hepatitis, and other infectious diseases for people experiencing homelessness or housing instability.
