Adds a new religious belief exemption to Minnesota's immunization requirements, allowing parents or guardians to submit a signed statement citing a "genuine and sincerely held religious belief" to exempt their child from required immunizations for school or child care facility enrollment.

Requires that written information about immunization exemptions be provided to persons being immunized or enrolling in schools or child care facilities in the same font size and on the same page as immunization requirements.

Modifies reporting requirements for schools and child care facilities to separately track exemptions granted under medical reasons, conscientiously held beliefs, and the new religious belief exemption category.

Requires immunization providers to supply written information listing required immunizations, descriptions of available exemptions, recommended immunizations, and federal vaccine information sheets before administering vaccines.

Directs the commissioner of health to continue educational campaigns to providers on vaccine safety, adverse event reporting systems, and vaccine contraindications, and to encourage use of thimerosal-free vaccines when available.
