Replaces the pilot project model with mandatory "adult mental health initiatives" that each county board must provide or contract for to deliver mental health services to adults with serious and persistent mental illness.

Requires adult mental health initiatives to be designed, planned, improved, and maintained by county boards to provide expanded service arrays, improve access through purchasing strategies, incorporate state facilities into community infrastructure, and creatively use existing funding streams.

Authorizes the commissioner of human services to facilitate fund integration, award grants for start-up and implementation costs, waive incompatible administrative rules, and review initiative status biennially with legislative recommendations by January 15 of odd-numbered years.

Mandates the commissioner to ensure no adult mental health initiative region receives less funding in fiscal year 2023 and beyond than it received in fiscal year 2022 when implementing a reformed funding formula.

Appropriates unspecified general fund dollars in fiscal year 2023 to the commissioner to guarantee no funding reductions result from changes to the adult mental health initiative funding formula.
