Requires all health plans sold, issued, or renewed to Minnesota residents to provide coverage for lymphedema treatment, including compression garments, stockings, sleeves, compression devices, and bandaging systems and supplies.

Mandates coverage for complex decongestive therapy and outpatient self-management training and education for lymphedema treatment when prescribed by a licensed health care professional.

Allows health carriers to require prescribing providers to be within the enrollee's health plan network if the network meets adequacy requirements under Minnesota Statutes section 62K.10.

Prohibits health plans from applying cost-sharing, benefit limitations, or service limitations for lymphedema treatment that place greater financial burden on enrollees or are more restrictive than limitations applied to other similar services or benefits.

Effective date of August 1, 2021, for any health plan issued, sold, or renewed on or after that date.
