Establishes a fee-waived identification card or driver's license process for persons eligible for medical assistance who have received substance use disorder treatment within the last 12 months.

Requires applicants to submit a signed application, a self-signed statement of eligibility and treatment history, and a verification statement from a publicly-funded human services agency employee.

Verification statement must include the verifying individual's name, address, phone number, and email; the applicant's name; and confirmation of medical assistance eligibility and substance use disorder treatment within the past 12 months.

Prohibits the commissioner from imposing fees, surcharges, or filing fees for identification cards issued under this process.

Prohibits driver's license agents from imposing filing fees for driver's licenses issued under this process.
