Converts "pilot projects" language to mandatory "adult mental health initiatives" requiring each county board to provide or contract for mental health services infrastructure for adults with serious and persistent mental illness under the Minnesota Comprehensive Adult Mental Health Act.

Establishes county boards as responsible for designing, planning, improving, and maintaining mental health service delivery systems that provide expanded service arrays, improve access through purchasing strategies, integrate state facilities into community infrastructure, and creatively combine existing funding streams.

Authorizes the commissioner to facilitate fund integration, award start-up and implementation grants, waive incompatible administrative rules, and exempt counties from fiscal sanctions; requires biennial reporting on funding use and outcomes to legislative committees.

Adds new subdivision 11 requiring that when implementing reformed funding formulas, no adult mental health initiative region receives less funding in fiscal year 2023 than it received in fiscal year 2022 from combined adult mental health initiative and Moose Lake Alternative funding.

Appropriates unspecified general fund dollars for fiscal year 2023 to ensure no adult mental health initiative region experiences funding reductions from the formula changes.
