Amends identification card application rules to allow Department of Corrections or Federal Bureau of Prisons identification cards as secondary proof of identity documents.

Requires the commissioner of corrections to provide released inmates with criminal history information, voting rights information, workforce center information, medical summaries, Social Security card assistance, and SNAP benefits information upon release.

Mandates the commissioner to assist inmates in obtaining birth certificates and state identification cards at no cost, except for inmates with six months or less remaining in their sentence or who already have valid identification.

Requires the commissioner to provide released inmates with a one-month supply of non-narcotic medications and refillable 30-day prescriptions, and to offer assistance with medical assistance or MinnesotaCare applications at least 45 days before release.

Directs the commissioner to develop a homelessness mitigation plan by October 31, 2022, and submit annual reports beginning February 15, 2022, detailing the number of individuals released to homelessness, disaggregated by race, gender, and county.
