Requires all health plans providing maternity benefits to cover infertility diagnosis and treatment procedures including ovulation induction, artificial insemination, in vitro fertilization, and fertility preservation, if medically necessary and recognized by the American Society for Reproductive Medicine or American College of Obstetrics and Gynecologists.

Limits coverage to unlimited embryo transfers but allows a maximum of four completed egg retrievals per plan year for private health plans.

Requires cost-sharing for infertility coverage (copayments, deductibles, coinsurance) to be no greater than cost-sharing for maternity coverage.

Adds infertility treatment coverage to Minnesota Medical Assistance, effective January 1, 2025, or upon federal approval, with specific provisions for fertility preservation services for cancer patients and other medical conditions.

Excludes coverage for surgical reversal of elective sterilization and donor gamete expenses; does not apply to health plans under state Medicaid or MinnesotaCare programs.
