Requires municipalities, businesses, nonprofit organizations, and schools organizing youth athletic activities with fees to require participating athletes to submit a medical eligibility form signed by a health care provider.

Medical eligibility form must document that the youth athlete has been screened for conditions associated with sudden cardiac arrest and assessed using a double-cuff blood pressure measurement device.

Maintains existing concussion protocol requirements including coach and official training on concussion recognition and management, with training required initially and every three years thereafter.

Coaches and officials must remove youth athletes from activities if they exhibit signs, symptoms, or behaviors consistent with a concussion, with return to play requiring evaluation by a trained provider and written permission.

Does not create additional liability for municipalities, businesses, nonprofit organizations, or their officers, employees, or volunteers.
