Requires a new nursing home license when the licensee's legal entity structure is converted or changed to a different type of legal entity structure.

Requires a new nursing home license when the licensee dissolves, consolidates, or merges with another legal organization and the licensee's legal organization does not survive.

Requires a new nursing home license when 50 percent or more of the licensee's ownership interest is transferred within the previous 24 months to a different person or multiple persons, or to persons who had less than 5 percent ownership interest at the time of the first transaction.

Requires a new nursing home license when any other event or combination of events results in a substitution, elimination, or withdrawal of the licensee's responsibility for the facility.

Authorizes the commissioner of health to prescribe licensure procedures by rule.
