Adds ITIN as an acceptable form of identification for individuals applying to manufacturer patient assistance programs for ongoing insulin coverage.

Maintains requirement that individuals under age 18 must have a parent or legal guardian provide proof of residency when using ITIN as identification.

Applies the ITIN identification option to both the urgent-need insulin program (30-day supply) and the continuing manufacturer patient assistance program (long-term coverage).
